This form is Part 1 of a 2 part registration process. It is to be used as part of the Balga SC player pre-registration.

This form will ask you to select your team and provide us with your details including payment details if you select Credit Card. For those registering Juniors, it will also ask for their full name, address, date of birth and contact details.

- Club Group. This is made up of helpers in the administrative area, Eg helping others register their children, help coordinate fundraising efforts & help organise events through the year.
